WELCOME
TO THE PRACTICE
We
are a six-doctor practice covering a large area of the Vale of Glamorgan. Our
main base is at the Eryl surgery in Llantwit Major which opened in July 1984,
when we moved from Hillbrow next to the Town Hall. The premises have been extended
several times, most recently in 2002 when an upstairs waiting area and three
consulting rooms were built. We also have modern purpose-built branch surgeries
in Rhoose and St Athan. All our premises have easy local parking and wheelchair
access.
Eryl Surgery is a training practice which means we have doctors attached to the
practice for six month periods. Registrars (doctors in training for general practice)
are medically qualified and many have a great deal of hospital experience, their
training in the practice being focused on the role of the general practitioner.
They are available for consultations in exactly the same way as the other doctors
in the practice. On occasions patients may be asked for their permission to video
record their consultation. Video recording will only be undertaken with patients' consent and the camera switched off on request.

THE RIGHT OF ACCESS TO PATIENT INFORMATION
The Access to Health Records Act 1990 gave individuals the right
of access, subject to certain exceptions, to health information recorded about
themselves, and, in certain circumstances, about others, within manual records.
The practice policy on Access to Health Records can be obtained from reception.
PATIENT RIGHTS AND RESPONSIBILITIES
Detailed information
on patient's rights and responsibilities
is available in The Patient's Charter and the White Paper "Putting
Patients First". Patients
are encouraged to take personal responsibilities for their own health and for
dealing with minor ailments. A doctor should only be called out in circumstances
of real need. Patients have a responsibility to turn up for appointments or to
advise the health service in good time if they cannot attend.
